Certification Work Sheets

These worksheets are the individual pagesof the forms that you need to record your certification progress on, including Levels 1 through 5, the Currency Requirement form, and all the Endorsement Records. We have also included the Facility Report, [ARES 403] and the Transit Report, [ARES 404] since these forms are used frequently in completing Level Certifications. They should also be filled out for any time an operator is assigned to EOCs and Hospitals.

We suggest that you save the worksheets as MS Word documents to disk as files. From there you can enter data on them and then print them for your Certification Record.

These Worksheets are also available in PDF Format. This Format doesn't allow manipulation of the data like the MS Word Documents, but you can print them and enter data using a pencil or pen.

PDF files can be printed quite easily but I suggest you use the printer link on the PDF form's toolbar (do not go to file and print).
